What is a claim?

In health insurance, a claim is a detailed invoice in the medical billing process.

In most cases, a health care provider sends a claim to a health insurance company or government insurance program for health care services that were performed on behalf of the health plan member. The health insurance company will process the claim according to industry standards and technical protocols in order to determine the reimbursement to the health care provider.
